Patient Resources

Everything you need to get started.

New to the practice? You'll find everything here — from what to bring to your first appointment, to how telehealth works, to helpful mental health resources.

New Patients

Before your first appointment.

Getting started is simple. Here's what to expect before we meet for the first time.

01

Request an appointment

Reach out through the contact page or call us directly. We'll confirm availability and send you a few short intake questions to complete before your visit.

02

Complete your intake forms

You'll receive a secure link to complete your new patient intake paperwork online — at your own pace, before your appointment. This helps us make the most of our time together.

03

Gather your health history

If you have records from previous providers, a list of current medications, or notes about past treatments, bring those along. The more context I have, the better I can help.

04

Prepare for your appointment

For telehealth visits, you’ll receive a secure video link before your appointment and should plan to join from a private, comfortable location. For in-person visits, you’ll receive office location and arrival information before your appointment. Either way, we’ll take it from there.

Understanding your coverage.

Common Questions

Things people often wonder.

Your initial evaluation is typically 60–90 minutes. I want to take the time to truly understand your history and what you're hoping for — not rush through a checklist.

A list of any current medications (including supplements), any relevant medical or psychiatric records if you have them, your insurance card, and any questions you'd like to ask. Most importantly — just yourself.

Follow-up frequency depends on your individual needs and treatment plan. Initially, appointments may be every 2–4 weeks. As you stabilize, we may move to monthly or quarterly check-ins.

Prescribing decisions are made on a case-by-case basis, guided by clinical need and your full history. I follow all state and federal regulations regarding controlled substances.

Please give at least 24 hours notice when possible. Late cancellations or no-shows may be subject to a fee. Life happens — just reach out as soon as you know.

Yes. The patient portal is available for non-urgent questions and updates between appointments.

Portal messages are not monitored continuously and should never be used for urgent or emergency situations. If you are experiencing a mental health emergency, call 911, go to your nearest emergency department, or call or text 988 for immediate support.
